EARLY DEPARTURE FORM And OFF-SITE AUTHORIZATION FORM


This form is to be completed by Parents / Caregivers only.

Please make sure that you read and understand the information on this page before completing the form. Forms MUST be completed by June 15, 2017 to be considered.

Early Departure:

Athletes are permitted to travel home with their parents/guardians after the games.  Athletes may depart with their families:

Provincial Winter Games - On Sunday, February 3 before 9:00 am.

Provincial Floor Hockey Championships - On Friday, May 17, before 9:00 am. 

Provincial 5-Pin Bowling Championships - TBD

Athletes can be picked up at the front door of their residence.

Please complete the online form using the link at the bottom of the page to confirm that the athlete will be traveling home with the parents / guardians.

Off – Site Authorization

Family members may wish to take athletes off site for dinner or sight seeing at some point during the Games.  Coaches and family members should know that all athletes must take part in the Opening Ceremonies, Closing Ceremonies, Family Dinner and all competitive activities.  Although we are pleased athletes have family support, please remember that they are part of a Team and as such have responsibilities.  Only those persons authorized on this form may take athletes off-site during the Games.  The Head Coach shall determine the time at which the athlete may leave the team and at which time he/she must be returned.  All authorized persons will be required to sign the athlete out and back in with the Head Coach using a form which indicates the time and location of return, how they can be contacted if necessary, etc.

In order to give particular persons authorization to take an athlete off-site while at the Games, please click on the link at the bottom of this section to complete the online form.  Forms must be completed by June 15, 2016.  If the form is not received by this date, the athlete will not be permitted to leave the Team while at the Games.

Although this may seem very strict to some parents and family members, please remember, these measures are in place for the safety of the athlete.

Please Remember: 

1.       Athletes are NOT permitted to consume alcoholic beverages while offsite.

2.       The teams have a set curfew for all athletes.  Offsite athletes must abide by that curfew as well.

3.       Athletes will not leave the team until the sign out form has been completed with the mission staff.
